I learned this 'sandwich' method in Toastmasters, where they always call reviews 'evaluations'. 'Where you were strongest' 'One weak area that could be improved' 'What I personally got out of it.'

The jet ski photo and intro is quite eye-catching and engaging. Although he/she is clunky, I either write gender neutral, or alternate references to male or female writers. Love how you illustrated point #4 by asking for feedback. Brilliant!
